Rehabilitation Counselor

JOB DESCRIPTION:

MAJOR DUTIES AND RESPONSIBILITIES:

  • Provide both day program and employment services to individuals with mental health disorders and/or development disabilities. Assist individuals in the day program to develop their independent living skills and/or the skills necessary to obtain competitive employment in their community. Provide transportation for day program, employment, or appointments in agency vehicle. De-escalate situations and implement interventions to develop positive coping skills, modify unhealthy habits, and manage mental health symptoms. Provide employment services to individuals within the employment program, assisting them to submit applications, interview with employers, negotiate positions, complete orientation, and provide long-term job coaching. Participate in regularly scheduled staff meetings for program and service planning. Responsible for maintaining all required chart work and documentation of all services. Complies with all requirements of OMH, OPWDD, ACCES-VR, and MEDICAID. Carries out other duties as assigned by Director of Day Program and Executive Director.

QUALIFICATIONS:
Bachelor’s Degree in Psychology, Human Services, Education or Related Field. Two years’ experience preferred working in the mental health field, with people with developmental disabilities preferred but not required. Must be of good character and display an affinity for working with people with disabilities and/or developmental disabilities. Must have the ability to work independently and as part of a multi-disciplinary team. Experience working with OMH, OPWDD, and/or ACCES-VR encouraged but not required. Must have a valid NYS driver’s license acceptable to Agency insurance standards. Local travel is required.

Full-Time, 40 hours/week

SCHEDULE:
Typical Hours are Monday-Friday 7am-3pm, or 8am-4pm.

PAY:
$36,000 – $41,000 Per Year
