ADULT CAREER AND CONTINUING EDUCATION SERVICES
FUNDED THROUGH CORE REHABILITATION SERVICES CONTRACT
- Diagnostic Vocational Evaluation– evaluates consumer skills within the day program or in a community locale via an enclave or mobile work crew. This evaluation is a vocational profile of the participant’s vocational strengths and weaknesses, capabilities, needs, skills and experience. The information gleaned from this evaluation assists in the development or refinement of the Individualized Plan for Employment.
- Work Readiness 1, 2, and 3 Services– Consumers are assisted by Liberty House staff at the program in development of job-related skills. Work Readiness Services include components that enable the consumer to successfully develop the following individual capacities for achieving and maintaining employment: work behaviors, social skills in the work setting, effective communication, accepting supervision, problem solving, grooming and hygiene, goal setting and work tolerance. Other areas of concern may also be addressed including: work-related daily living skills, disability awareness, work traits and work ethics in employment settings. The higher levels of this service also include development of vocational skill sets focused on specific fields of employment and utilized in a work experience.
- Supported Employment– Paid competitive employment in an integrated setting with on-going support for individuals with the most severe disabilities (i.e., psychiatric, mental retardation, significant learning disabilities, traumatic brain injury and other disabilities) for whom competitive employment has not traditionally occurred, and who, because of the nature and severity of their disability, need on-going support services in order to obtain, perform, and retain their job. Supported Employment provides assistance such as job coaching and job placement, assistance in interacting with employers, on-site assistive technology training, specialized job training, and individually tailored supervision. Supported Employment services provided in enclaves and mobile work crews are also provided. Supported Employment provides a vehicle to enable eligible individuals to enter into competitive employment where they would otherwise, due to the impact of their disabling conditions, be unable to do so. It is understood that the provision of on-going support services for people with severe disabilities significantly increases their rates for employment retention. Supported Employment encourages people to work within their communities and encourages working, social interaction, and integrations with their non-disabled peers.
- Job Placement Services– For individuals who do not meet the criteria for Supported Employment, direct job placement services are available. Liberty House staff will help the individual determine if there is an adequate match in vocational goal, employability and placement potential in a particular occupation. They will assist the individual through the job seeking, development and placement process. Staff will assist the individual with retention services for the initial 90 days of employment.
- Transportation– For eligible individuals ACCES-VR may assist with providing transportation to and from services provided by Liberty House.

FUNDED THROUGH MEDICAID HOME AND COMMUNITY BASED WAIVER
Liberty House is dedicated to providing person-centered plans and supports that promote the physical and emotional well-being of all our members. We do this with integrity, respect, justice and fairness with self-determination as our guiding principle.
- Prevocational Services: These services provide an opportunity for individuals who are interested in joining “the world of work”, the support and training necessary to obtain and retain competitive employment. Services may include: following directions, attending to task, task completion and problem solving. Services also focus on learning how to relate to co-workers and supervisors as well as productivity demands in the work place.
- Intensive Supported Employment: Liberty House provides services and interventions that are required to assist a person to successfully obtain and develop the skills to retain employment that is meaningful to them. Liberty House staff will work with the individual until stabilization of the job has occurred.
- Extended Supported Employment: Once stabilized, Liberty House provides staff who will meet with the individual at least twice monthly, at or away from the work site, as required to maintain the job.

NEW YORK STATE OFFICE OF MENTAL HEALTH
FUNDED THROUGH DIRECT CONTRACT WITH OFFICE OF COMMUNITY SERVICES FOR WARREN AND WASHINGTON COUNTIES
- Psychosocial Rehabilitation: People disabled by mental illness face extraordinary obstacles to achieving their personal goals. These services are designed to assist the individual in eliminating and/or compensating for functional deficits and barriers associated with their illness. At the day program an individual has an opportunity to enhance interpersonal skills, establish support networks, increase community awareness, develop coping strategies, and improve hygiene and grooming, to deal with the negative effects of their psychiatric symptoms and develop and enhance daily living skills.
- Prevocational Services/Day Training: These services are designed to prepare individuals for paid employment. Individuals participate in a variety of prevocational training units where they are taught concepts such as compliance, attendance, task completion, problem solving, safety in the workplace, reliability, dependability and symptom management.
- Intensive Supported Employment: Liberty House provides services and interventions that are required to assist a person to successfully obtain and develop the skills to retain employment that is meaningful to them. Liberty House staff will work with the individual until stabilization of the job has occurred.
- Ongoing Integrated Supported Employment: Once stabilized, Liberty House provides staff who will meet with the individual at least twice monthly, at or away from the work site, as required to maintain the job.
